[eng] Streptococcus pyogenes is a human pathogenic bacteria that causes a variety of infections, some of which are mild, such as pharyngitis or skin infections, such as impetigo, and other more serious diseases such as scarlet fever or streptococcal shock-toxic syndrome. In addition, this pathogen can cause post-infectious diseases, which develop once a previous infection has been overcome by the bacteria, such as rheumatic fever or post-streptococcal glomerulonephritis. Its diagnosis is based on the culture of the bacteria in blood agar plates. As well, other techniques such as Gram staining, catalase activity, bacitracin susceptibility or PYR test, can be performed for definitive identification, without forgetting the Lancefield A antigen detection test. To cause disease in the host, S. pyogenes possesses a great variety of virulence factors that through various pathogenicity mechanisms allow the bacteria to develop the infection. Many of them are involved in the process of adhesion and colonization (lipoteicoic acid), in the evasion of the immune system (hyaluronic acid capsule, C5a peptidase, M protein, SLO, SpeB) and in facilitating the propagation of the bacteria in the soft tissues (Streptokinase, Hyaluronidase, SLS). Treatment for this type of infections is mainly based on penicillin. Regarding vaccination, research is still under way to create an efficient and safe human vaccine against the pathogen. |
