[spa] Introducción: Entre el 28% y el 60% de niños con PC (Parálisis Cerebral) tienen afectación en
la cadera y su incidencia de DDC (Displasia del Desarrollo de la Cadera) se correlaciona con
la severidad y el estado ambulatorio del niño (dependiendo del nivel en la escala GMFCS). La
DDC en PC tiene dos principales causas: la asimetría producida por la espasticidad y el
desequilibrio de músculos que se insertan en la cadera, y la falta de carga.
Objetivo: Conocer la prevención desde el ámbito de la fisioterapia aplicada a niños con
parálisis cerebral y riesgo de desarrollar DDC.
Resultados: Se realizó una búsqueda bibliográfica y doce estudios fueron incluidos según los
criterios de inclusión y exclusión. Éstos hablan de los tratamientos posturales aplicados,
eficacia, screening de detección de la DDC, qué son cada unos de los conceptos y sus tipos.
Hay once estudios más que están relacionados, que explican y completan los apartados
propuestos.
Conclusión: Se centrará en la función preventiva del tratamiento. La prevención se hará
mediante programas de bipedestación, sillas de yeso, equipos de posicionamiento, sedestación
y bipedestación. Éstos han demostrado efectividad en niños con PCI y altos valores en la
escala GMFCS. Pero se incide en que no hay suficiente información sobre protocolos de
actuación, y en la necesidad de estudios para comprender el impacto de las diferentes
estrategias de prevención sobre su efectividad y la calidad de vida de los pacientes. |

[eng] Introduction: Between 28% and 60% of children with Cerebral Palsy (CP) have involvement
in the hip and their incidence of Developmental Dysplasia of the Hip (DDH) correlates with
the severity and ambulatory status of the child (GMFCS). DDH in CP has two main causes:
the asymmetry produced by spasticity and the imbalance of the muscles inserted in the hip,
and the lack of load.
Objective: To know the prevention from the field of physiotherapy applied to children with
cerebral palsy and with risk of DDH.
Results: A literature search was made and twelve studies were included according to the
inclusion and exclusion criteria. The postural treatments, effectiveness, screening for DDH,
the concepts and their types were studied. There are eleven studies that are related, they
explain and complete the proposed sections.
Conclusion: The prevention is done through standing programs, gypsum chairs, positioning
equipment, sitting and standing. These have had an effect on children with CP and high
values on the GMFCS scale. But it is argued that there is not enough information on protocols
for action, and the need for studies to understand the impact of different prevention strategies
on their effectiveness and the quality of life of the patients. |
