Efecto de una intervención nutricional en pacientes ingresados por fractura de cadera

[spa] La fractura de cadera debida a la osteoporosis, es una de las principales causas discapacitantes y que mayor pérdida de calidad de vida provoca en la población mayor. Los factores de riesgo con los que se relaciona son muy dispares, desde enfermedades sistémicas, sexo femenino, fracturas anteriores hasta malnutrición y discapacidad funcional. En España la incidencia de pacientes ingresados por fractura de cadera se estima en 551 casos por cada 100.000 habitantes mayores de 65 años. La mortalidad hospitalaria de este tipo de pacientes es de un 5,3%. La presencia de desnutrición o de riesgo nutricional en el momento de la fractura es un factor que se relaciona con un aumento de la estancia hospitalaria tras la intervención, así como con un aumento del riesgo de complicaciones. El objetivo de este estudio consiste en aplicar una intervención nutricional desde el primer día de ingreso del paciente, donde se recojan datos suficientes a través de herramientas de valoración tales como CONUT, Mini-Nutritional Assessment, y datos analíticos con los parámetros más relevantes (colesterol total, allbúmina y linfocitos totales, entre otros), para establecer el grado nutricional del mismo a su llegada al hospital, e iniciar una dieta apropiada que se ajuste a sus necesidades, y que se vaya modificando según sea necesario, para así acortar su estancia hospitalaria. Para ello, durante 6 meses, a todo paciente que ingrese en el Servicio de Traumatología y Ortopedia del Hospital Universitario Reina Sofía de Murcia, con diagnóstico de fractura de cadera, mayor de 65 años, desde el primer día de ingreso y durante su estancia, se le valorará y se le realizará un seguimiento nutricional con las herramientas anteriormente citadas, por parte del equipo multidisciplinar del servicio, dando así un mejor soporte nutricional al paciente.

[eng] Hip fracture due to osteoporosis is one of the main disabling causes and which produces the greatest loss of quality of life in the elderly population. The related risk factors are very different, ranging from systemic diseases, female gender, previous fractures to malnutrition and functional disability. In Spain, the incidence of patient admitted for hip fracture is estimated at 551 cases per 100.000 in habitants over the age of 65. The hospital mortality of this type of patients is 5,3%. The presence of malnutrition or nutritional risk at the time of fracture is a factor which is related to an increase in hospital stay after the operation, as well as to the increased risk of complications. The aim of this study is to apply a nutritional intervention from day one of the patient’s admission, where sufficient data are collected thorough the following tools such as CONUT, Mini-Nutritional Assessment and analytical data with the most relevant parameters (total cholesterol, albumin and total lymphocytes, among others), for establishing the nutritional status of the patients upon arrival at the hospital, and initiate an appropriate diet to suit their needs, and to be modified as necessary, in order to shorten their hospital stay. To this end, for 6 months, all patients admitted to the Traumatology and Orthopedics Service of the University Hospital Reina Sofía of Murcia, with diagnosis of hip fracture, over 65 years old, from the first day of admission and during their stay, they will be assessed and will be followed up nutritionally with the tools mentioned above, by part of the multidisciplinary team of the service, thus giving a better nutritional support to the patient.
