[spa] INTRODUCCIÓN Y OBJETIVOS: La desnutrición en pacientes con insuficiencia cardíaca
crónica es muy prevalente y condiciona mayor morbimortalidad. Se ha demostrado que la
suplementación nutricional consigue, a largo plazo, mejorar el pronóstico de esta patología.
No hay estudios que determinen el efecto de la suplementación nutricional oral en la
evolución clínica del paciente durante el ingreso hospitalario, y todos los estudios se han
realizado con suplementos orales hiperproteicos. El objetivo de este estudio es determinar
si una suplementación nutricional oral con alto contenido en hidratos de carbono y
normoproteica, iniciada durante un ingreso por insuficiencia cardiaca aguda, disminuye la
mortalidad y los días de estancia hospitalaria del propio ingreso y/o mejora la clase
funcional al alta.
MATERIAL Y MÉTODOS: El estudio es una investigación clínica con un producto sanitario.
En el mismo participarán los servicios de Medicina Interna y Cardiología del Hospital
Universitario Morales Meseguer (Murcia). Los sujetos serán una muestra de los pacientes
hospitalizados que hayan sido diagnosticados de insuficiencia cardiaca aguda (de novo o
descompensada) y que cumplan criterios de desnutrición (puntuación en la encuesta Mini
Nutritional Assessment menor de 17 o Controlling Nutritional Status mayor de 2). Los
pacientes de la muestra serán incluidos en dos grupos experimentales: control y grupo de
intervención. Los pacientes del grupo control recibirán tratamiento convencional para la
insuficiencia cardíaca aguda y un placebo en lugar de suplementación oral, por tanto, los
hidratos de carbono diarios serán los obtenidos únicamente de la dieta basal (5 g/kg
aproximadamente). El grupo de intervención, además del tratamiento de la insuficiencia
cardíaca aguda, recibirán 10 g/kg de hidratos de carbono al día para lo que se administrará
un suplemento nutricional oral compuesto por maltodextrina.
CONCLUSIONES: La realización de este proyecto ha de permitir conocer si la
suplementación nutricional oral con alto contenido en hidratos de carbono mejora la
evolución clínica del paciente ingresado por un episodio de insuficiencia cardíaca aguda.
[eng] INTRODUCTION AND OBJECTIVES: Malnutrition in patients with chronic heart failure is
very prevalent and conditions higher morbidity and mortality. It has been shown that
nutritional supplementation improves the prognosis of this pathology in the long term. There
are no studies that determine the effect of oral nutritional supplementation on the clinical
evolution of the patient during hospital admission, and all the studies have been carried out
with hyperprotein oral supplements. The objective of this study is to determine whether an
oral nutritional supplement with a high content of carbohydrates and normoprotein, initiated
during an admission for acute heart failure, reduces mortality and the days of hospital stay
of the admission itself and / or improves functional class upon discharge.
MATERIAL AND METHODS: The study is a clinical investigation with a medical device.
The Internal Medicine and Cardiology services of the Morales Meseguer University Hospital
(Murcia) will participate in it. The subjects will be a sample of hospitalized patients who
have been diagnosed with acute heart failure (de novo or decompensated) and who meet
the criteria for malnutrition (Mini Nutritional Assessment survey score less than 17 or
Controlling Nutritional Status greater than 2). The patients in the sample will be included in
two experimental groups: control and intervention group. The patients in the control group
will receive conventional treatment for acute heart failure and a placebo instead of oral
supplementation, therefore, the daily carbohydrates will be obtained only from the basal diet
(approximately 5 g/kg). The intervention group, in addition to the treatment of acute heart
failure, will receive 10 g/kg of carbohydrates per day for which an oral nutritional
supplement composed of maltodextrin will be administered.
CONCLUSIONS: Carrying out this project should make it possible to know if oral nutritional
supplementation with a high carbohydrate content improves the clinical evolution of the
patient admitted for an episode of acute heart failure.
