[spa] Introducción. La diabetes mellitus (DM) constituye la alteración metabólica que más
frecuentemente se asocia al embarazo afectando al pronóstico de la madre y al de su
futuro hijo. La diabetes gestacional se define en la Guía Asistencial de Diabetes
Mellitus y Embarazo como: toda diabetes mellitus diagnosticada por primera vez
durante el embarazo, independientemente de la necesidad de tratamiento insulínico,
grado del trastorno metabólico o su persistencia una vez finalizado el mismo. Este tipo
de diabetes es diferente a la diabetes pregestacional que se diagnostica antes de la
gestación y que se mantiene presente durante la misma. (1)
Entre el 7- 12% de las gestantes tienen riesgo de desarrollar esta enfermedad a lo largo
del embarazo, este intervalo diagnóstico depende de la estrategia diagnóstica empleada
durante el embarazo. El sobrepeso previo al embarazo es uno de los factores de riesgo
para el desarrollo de diabetes gestacional. (2)(3)
El tratamiento de la DM ha demostrado disminuir las complicaciones maternas y
perinatales, por lo que su diagnóstico según la Sociedad Española de Obstetricia y
Ginecología (SEGO) está justificado.
Hipótesis. La adhesión a una intervención de control dietético y hábitos saludables al
inicio de la gestación reducirá en las gestantes con obesidad el riesgo de desarrollar
diabetes gestacional a lo largo del embarazo.
Objetivo. Analizar el posible efecto beneficioso de una mejora en los hábitos saludables
(dieta y ejercicio) en gestantes con riesgo de diabetes gestacional al inicio de embarazo,
y la disminución de la incidencia de la enfermedad durante el embarazo.
Metodología. Ensayo clínico aleatorizado con una muestra de 317 mujeres
embarazadas obesas, con un Índice de Masa Corporal (IMC) igual o superior a 30
kg/m2
, que no hayan sido diagnosticadas durante el primer trimestre de diabetes
gestacional; 158 de estas mujeres, de forma aleatoria, pasarán a formar parte del grupo
control y las 158 mujeres restantes al grupo de intervención. El grupo control recibirá la
atención habitual en la consulta y con el grupo de intervención se llevará a cabo la
intervención dietética y de cambios de vida más saludables.

associated with pregnancy, affecting the prognosis of the mother and her future child.
Gestational diabetes is defined in the Guidelines on Diabetes Mellitus and Pregnancy as
all diabetes mellitus diagnosed for the first-time during pregnancy, regardless of the
need for insulin treatment, the degree of the metabolic disorder, or its persistence after
the end of pregnancy. This type of diabetes is different from pregestational diabetes that
is diagnosed before pregnancy and remains present during pregnancy. (1) Between 7-
12% of pregnant women are at risk of developing this disease during pregnancy, this
diagnostic interval depends on the diagnostic strategy used during pregnancy. Prepregnancy overweight is one of the risk factors for the development of gestational

pregnancy in pregnant women with obesity.
Objective. To analyze the possible beneficial effect of an improvement in healthy
habits (diet and exercise) in pregnant women at risk of gestational diabetes at the
beginning of pregnancy, and the decrease in the incidence of the disease during
pregnancy.
Methodology. A randomized clinical trial with a sample of 317 pregnant women with
obesity (BMI equal to or greater than 30) who have not been diagnosed during the first
trimester of gestational diabetes;158 of these women will be randomly assigned to the
control group, and the remaining 158 to the intervention group. The control group will
receive the usual care in the office and the intervention group will receive the dietary
intervention and healthier lifestyle changes.
