Condicionantes de la decisión de morir en casa para el paciente terminal y su familia

[spa] Introducción: No hace mucho tiempo el cuidado de los seres queridos al final de la vida era tarea casi exclusiva de las familias, hasta que a mediados del siglo XX vivimos un cambio de paradigma en el que se traspasa a las instituciones y se profesionaliza. Hoy en día el hospital es el lugar en el que fallecen la mayoría de las personas; si bien, una gran parte preferirían hacerlo en su domicilio. Para la evidencia no hay un lugar mejor que otro, aunque sí defiende que respetar el entorno preferido es un indicador de calidad de los cuidados paliativos. Objetivo: Analizar los condicionantes que determinan que un paciente terminal muera o no en su domicilio, identificando además los principales beneficios e inconvenientes descritos por la literatura. Método: Responde a una estrategia de búsqueda bibliográfica sobre el fenómeno a través de la consulta de cuatro bases de datos en ciencias de la salud: Pubmed, Ibecs, Cinahl y Scielo; donde se seleccionaron 27 artículos que fueron analizados. Resultados: Los condicionantes de la decisión de la localización de la muerte se pueden agrupar en factores individuales, relacionados con la enfermedad y/o ambientales. Y la literatura describe beneficios y repercusiones negativas tanto para el paciente como para su familia en el hecho de llevar a cabo la muerte en el domicilio. Conclusiones: En general, la evidencia señala la importancia de adecuar la discusión sobre el lugar de la muerte a las preferencias y contexto individuales de cada paciente y familia.

[eng] Background: Not long ago the care of loved ones at the end of life was almost exclusive task of the families, until in the middle of the twentieth century we experienced a paradigm shift in which it was transferred to institutions and professionalized. Today the hospital is the place where most people die; although a large majority would prefer to do so at home. For the evidence there is no better place than another, although it does argue that respecting the preferred environment is an indicator of the quality of palliative care. Aim: To analyze the conditions that determine whether or not a terminal patient dies at home, also identifying the main benefits and disadvantages described by the literature. Methods: It responds to a bibliographic search strategy on the phenomenon through the consultation of four databases in health sciences: Pubmed, Ibecs, Cinahl and Scielo; where 27 articles were selected and analyzed. Results: Conditional determinants of the location of death can be grouped into individual, disease related and/or environmental factors. And the literature describes benefits and negative repercussions for both the patient and his family in carrying out death at home. Conclusions: In general, the evidence points to the importance of adapting the discussion on the place of death to the individual preferences and context of each patient and family.
