[spa] Cuando se produce una quemadura, si la extensión de ésta alberga entre un 25
a un 30% o superior de la superficie corporal total, se producen efectos a nivel
sistémico. Tres son los posibles procesos fisiopatológicos que pueden desencadenarse:
shock post quemadura, respuesta inflamatoria y respuesta hipermetabólica. Los
fluidos y electrolitos se ven alterados, el riesgo de infección se multiplica, y los cambios
metabólicos conllevan una serie de complicaciones cuyas repercusiones pueden
afectar a órganos vitales.
A lo largo de los últimos 40 años se ha estado estudiando diversas
intervenciones nutricionales y farmacológicas capaces de modular la respuesta inmune
y metabólica. Éstas han probado su eficacia en la disminución de la malnutrición
aguda, la modulación de la respuesta inflamatoria y la mejora de los resultados
clínicos.
Nuestro objetivo de estudio será determinado principalmente por la capacidad
de poder menguar aquellos efectos sistémicos provocados por las quemaduras graves,
y sus repercusiones. Para ello se realizará una guía de intervención nutricional, la cual
será individualizada por paciente. Como parte de dicha intervención nutricional, se
valorarán métodos de control (controles analíticos, de peso, balance nitrogenado,
etc.), así como la idoneidad de las distintas vías de administración (enteral /
parenteral) o la posible inclusión de suplementos nutritivos.
Para la implantación del presente proyecto se llevarán a cabo los siguientes
criterios de inclusión: pacientes ingresados en la unidad de quemados mayores de 16
años, con un porcentaje de superficie corporal quemada mayor de un 25%, y con
quemaduras de 2º o 3º grado. Se planteará el proyecto como un estudio de casos y
control, por lo tanto, los pacientes con estas características a los que se les aplique la
“guía resultante del estudio” formarán el grupo de “casos”, pudiéndose comparar
prospectivamente con aquellos otros a los que se les realiza el tratamiento estándar,
grupo de control.
Con la futura implantación del proyecto nutricional en las unidades de
quemados se espera poder reducir los niveles de infección, y complicaciones asociadas
a este tipo de pacientes, así como disminuir los tiempos de recuperación o incluso
incrementar la supervivencia de estos. |

[eng] When a burn occurs, if the extension of the burn lodges between 25 to 30% or more of
the total body surface, effects are produced at a systemic level. There are three possible
pathophysiological processes that can be triggered: shock post burn, inflammatory response
and hypermetabolic response. The fluids and electrolytes are altered, the risk of infection
multiplies, and the metabolic changes involve a series of complications whose repercussions
can affect vital organs.
Over the last 40 years, various nutritional and pharmacological interventions capable
of modulating the immune and metabolic response have been studied. These have proven
effective in reducing acute malnutrition, modulating the inflammatory response and improving
clinical outcomes.
Our study objective will be determined mainly by the ability to diminish those systemic
effects caused by severe burns, and their repercussions. For this, a nutrition intervention
guide will be made, which will be individualized by patient. As part of this nutritional
intervention, control methods (analytical controls, weight control, nitrogen balance, etc.) will
be evaluated, as well as the suitability of the different administration routes (enteral /
parenteral) or the possible inclusion of nutritional supplements.
For the implementation of this project the following inclusion criteria will be carried
out: patients admitted to the burn unit older than 16 years, with a percentage of burned body
surface area greater than 25%. The project will be considered as a case and control study,
therefore, patients with these characteristics to whom the "guide resulting from the study"
will be applied will form the group of "cases", being able to compare prospectively with those
others that they are given the standard treatment, control group.
With the future implementation of the nutritional project in the burn units is expected
to reduce the levels of infection, and complications associated with this type of patients, as
well as reduce recovery times or even increase the survival of these. |
