Evolución del estado nutricional en pacientes con insuficiencia renal crónica avanzada tras inicio de tratamiento con diálisis peritoneal

[spa] Introducción: La Diálisis Peritoneal (DP) es uno de los tratamientos renales sustitutivos con los que contamos. Los pacientes en DP tienden a la malnutrición, por tanto, es fundamental un diagnóstico precoz. El nefrólogo a su vez debe poner medidas para evitar su aparición, pero la actitud del paciente puede ser determinante para conseguir un apropiado estado nutricional. Hipótesis: Es posible evitar la tendencia a la desnutrición de los pacientes en DP mediante la aplicación de consejos nutricionales de forma regular e individualizada, durante el primer año de tratamiento sustitutivo. Objetivo: Valorar la evolución del estado nutricional en pacientes incidentes en DP durante un año, a los que se les aplica recomendaciones nutricionales para evitar la propensión a la desnutrición, empleando para ello parámetros antropológicos, bioquímicos, valoración de ingesta proteica (nPCR) y Bioimpedancia Espectroscópica (BIS). Material y método: Estudio propectivo intervencionista. Incluirá todos lo pacientes que inicien DP en los Hospitales de Albacete y Cuenca entre el 1 de Octubre de 2017 y el 1 de Octubre de 2018. Se asignarán los pacientes a dos grupos de manera aleatorizada: grupo de intervención (GI), al que se le aplicarán recomendaciones nutricionales y Grupo Control (GC). Se valorará estado nutricional mediante parámetros antropométricos, bioquímicos, nPCR y BIS cada 3 meses a lo largo de un año. Se comparará la evolución del estado nutricional entre GI y GC. Plan de trabajo: Previa aprobación del estudio por el Comité Ético de Investigación Clínica y bajo consentimiento informado, se iniciará el reclutamiento de pacientes entre el 1 de Octubre de 2017 y el 30 de Septiembre de 2018. La recogida de datos se realizará entre el 1 de Octubre de 2018 y el 30 de Septiembre de 2019. Finalmente se procederá al análisis de resultados en los 3 meses sucesivos (del 1 de Octubre al 31 de Diciembre de 2019).

[eng] Background: Peritoneal Dialysis (PD) is one of the renal replacement treatment we have. PD patients tend to malnutrition so an early diagnosis is essential. The nephrologist should use strategies to prevent their appearance, but the patient´s attitude can be determinant to achieve an appropriate nutritional status. Hypothesis: It is possible to avoid the tendency to malnutrition of PD patients by applying nutritional advice on a regular and individual manner during the first year of replacement treatment. Objective: To evaluate the evolution of nutritional status in PD patients during a year, who are given nutritional recommendations to avoid propensity to malnutrition, using anthropological parameters, biochemical, protein intake measurement (nPCR) and Bioimpedance Spectroscopic (BIS). Material and Methods: Interventional prospective study. It will include all patients who initiate PD in Albacete and Cuenca Hospitals between October 1, 2017 and October 1, 2018. Patients will be assigned to two groups in a randomized manner: Intervention Group (IG), which will receive nutritional advice and Control Group (GC). Nutritional status will be assessed using anthropometric parameters, biochemical, nPCR and BIS parameters every 3 months over a year. The evolution of the nutritional status between GI and GC will be compared. Work plan: After the approval by the Ethical Committee of Clinical Research and under informed consent, the recruitment of patients will begin between October 1, 2017 and September 30, 2018. Data collection will take place between October 1 2018 and September 30, 2019. Finally, the results will be analyzed in the next 3 months (from October 1 to December 31, 2019).
