[spa] Introducción: No hay un consenso sobre el tratamiento de la parálisis braquial obstétrica, aunque muchos autores resaltan los beneficios de una terapia eminentemente conservadora. Hay evidencias de que el “no uso aprendido” provoca una disminución de las representaciones de las áreas motoras en la corteza cerebral, como consecuencia de la no utilización del miembro afecto. El tratamiento mediante la terapia de restricción parece tener beneficios en pacientes con PBO debido a la plasticidad neuronal y reorganización cortical que se produce tras el periodo de intervención.
Objetivo: Determinar la evidencia que tiene la terapia de inducción por restricción del movimiento en la recuperación funcional en la parálisis braquial obstétrica.
Metodología: Tras la realización de la búsqueda bibliográfica a través de Pubmed, PEDro ScienceDirect y BVS con las palabras clave: Obstetric brachial plexus palsy, Constraint induced movement therapy, OBPP y CIMT, se consiguieron 4 artículos para dicha revisión, 3 de ellos se obtuvieron de Pubmed y 1 en la base de datos ScienceDirect.
Resultado: Analizando los resultados de forma individual todos los artículos parecen tener efectos positivos en los niños con PBO mediante la terapia de inducción por restricción del movimiento (en inglés, CIMT).
Conclusión: Se necesitan más estudios que demuestren los beneficios de la CIMT en la parálisis braquial obstétrica, debido al reducido número de ensayos clínicos existentes. |

[eng] Introduction: There is no consensus on the treatment of obstetric brachial plexus palsy, although many authors highlight the benefits of an eminently conservative therapy. There is evidence that the "learned nonuse" causes a decrease in the representations of the motor areas, both in the cortex and in the marrow, as a consequence of the non-use of the affected limb. Treatment by restriction therapy seems to have benefits in patients with OBPP due to neuronal plasticity and cortical reorganization that occurs after the intervention period.
Objective: To determine the evidence of induction therapy due to the movement restriction in functional recovery in obstetric brachial palsy.
Methodology: Databases of Pubmed, PEDro, ScienceDirect and BVS were searched using the following terms: Obstetric brachial plexus palsy, Constraint induced movement therapy, OBPP and CIMT. A total of four articles were considered potentially relevant for this review. Three of them were obtained from Pubmed and the other one from ScienceDirect.
Results: Analyzing the results individually, all the articles seem to have positive effects in children with OBPP treated through constraint induced movement therapy (CIMT).
Conclusion: Further studies are needed to demonstrate the benefits of the CIMT in obstetric brachial palsy due to the small number of existing clinical trials. |
