[spa] El melanoma es el tipo de cáncer de la piel más agresivo que existe debido a su capacidad diseminativa hacia los nódulos linfáticos y otras partes del cuerpo, causando así metástasis. Este tipo de tumor se origina cuando los melanocitos se transforman a células cancerosas como consecuencia de la aparición de mutaciones en oncogenes y genes que regulan la supervivencia y proliferación celular. En un 80% de los casos de melanoma si se extirpa el lunar o mancha maligna tempranamente el paciente queda libre de la enfermedad. Sin embargo, cuando el melanoma ya se encuentra en estado avanzado o metastásico (MM) es inoperable y no responde de manera eficaz a los tratamientos con fármacos quimioterapéuticos convencionales. El descubrimiento de varias mutaciones que desencadenan el melanoma ha permitido el desarrollo y aprobación de terapias dirigidas que mejoran el estado de la enfermedad y alargan la supervivencia de los pacientes con melanoma avanzado. Asimismo, la elucidación de los mecanismos de evasión al sistema inmune de los que hacen uso las células cancerosas ha permitido el desarrollo y aprobación de varios fármacos inmunoterapéuticos, que han resultado ser eficaces contra el melanoma en estado avanzado. Por este motivo, el objetivo de este trabajo bibliográfico fue la comparación de la efectividad de los fármacos de terapia convencional con los fármacos inmunoterapéuticos contra melanomas en estado avanzado o metastásico. De esta manera se observó que las terapias con agentes inmunológicos son mucho más eficaces que las terapias con agentes quimioterapéuticos debido a su mayor especificidad. No obstante, aún se trabaja en el estudio de combinaciones y dosis no tóxicas para poder administrar estos fármacos como tratamiento de primera línea en MM y dejar atrás la quimioterapia convencional. |

[eng] Melanoma is the most aggressive type of cancer that exists due to its disseminative capacity towards the lymph nodes and other parts of the body, causing metastasis. This type of tumor originates when the melanocytes are transformed into cancerous cells as a consequence of the appearance of mutations in oncogenes and genes that regulate cell survival and proliferation. In 80% of cases of melanoma if the mole or malignant spot is removed early, the patient will be free of the disease. However, when the melanoma is in an advanced or metastatic state (MM) it is inoperable and does not respond effectively to treatments with chemotherapeutic drugs. The discovery of several mutations that trigger melanoma has allowed the development and approval of targeted therapies that improve the disease status and survival of patients with advanced melanoma. Furthermore, the elucidation of the mechanisms of evasion of the immune system by cancerous cells has allowed the development and approval of several immunotherapeutic drugs, which have proved to be effective against advanced melanoma. For this reason, the objective of this bibliographic work was the comparison of the efficacy between the conventional therapy drugs and the immunotherapeutic drugs against advanced melanomas. In this case, the data confirmed that therapies with immunological agents are much more effective than therapies with chemotherapeutic agents due to their greater specificity. However, work is still being done on the study of combinations and non-toxic doses in order to administer these drugs for MM as a first-line treatment and thus leave behind conventional chemotherapy. |
