[cat] Una bacterièmia es defineix com la presència de bacteris a la sang, fet que es posa de manifest per
l’aïllament d’aquests a l’hemocultiu. El seu origen pot ser divers en funció de les característiques clíniques
del pacient, però si aquests bacteris envaeixen el torrent sanguini i es multipliquen a un ritme superior que
la capacitat del sistema reticuloendotelial té per eliminar-los, o es troben dintre d’un pacient amb el sistema
immunitari deficient, la bacterièmia pot donar lloc a infeccions greus i en alguns casos arribar a produir una
septicèmia, que desencadenaria una resposta més generalitzada. Encara que el coneixement d’aquesta
afecció ha anat creixent amb els anys, la mortalitat per sèpsia segueix sent una causa molt comú de mort
en tot el món. Un tractament antibiòtic empíric adequat s’associa amb una reducció significativa de la
mortalitat, sobretot amb pacients que pateixen malalties de base i a centres hospitalaris amb elevades taxes
de microorganismes multi-resistents. És de gran importància estudiar nous mètodes d’identificació i
determinació de sensibilitat antibiòtica, per poder obtenir un diagnòstic el més aviat possible, ja que de
forma convencional la identificació dels hemocultius positius requereix almenys de 24 hores d’incubació i
l’estudi de sensibilitat de 48 hores. Per això, en aquest present treball volem valorar l’eficàcia de dos nous
protocols, el primer del quals permet reduir el temps d’identificació, per obtenir-la el més aviat possible, i el
segon obtenir el perfil de sensibilitat, per poder aconseguir d’aquesta manera un diagnòstic més precoç per
un correcte tractament. El mètode d’identificació ha mostrat un 97 % d’efectivitat en bacteris Gram negatius
i un 73 % en bacteris Gram positius. L’efectivitat antibiòtica en enterobacteris ha estat superior al 79 % i en
S. aureus superior al 50 %.
[eng] A bacteraemia is defined as the presence of bacteria in the blood, which is evidenced by the isolation of
these to the blood culture. Its origin can be diverse depending on the clinical characteristics of the patient,
but if these bacteria invades the bloodstream and multiply at a higher rate than the ability of the
reticuloendothelial system has to eliminate them or are found within a patient with the immune system
deficient, bacteraemia can lead to serious infections and in some cases, lead to septicaemia, which would
trigger a more widespread response. Although knowledge of this condition has been growing with over the
years, mortality from sepsis remains a very common cause of death worldwide. An adequate empirical
antibiotic treatment is associated with a significant reduction of mortality, especially with patients suffering
from base diseases and hospital centres with high rates of multi-resistant microorganisms. It is of great
importance to study new methods of identification and the determination of antibiotic sensitivity, in order to
obtain a diagnosis as soon as possible, since conventionally the identification of positive blood cultures
requires at least 24 hours of incubation and the sensitivity study of 48 hours. For this reason, in this present
work we will evaluate the effectiveness of two new protocols, the first one allows to reduce the identification
time, to obtain it as soon as possible, and the second one to obtain the sensitivity profile, in order to get an
earlier diagnosis for a correct treatment. The method of identification has shown a 97 % effectiveness in
bacteria Gram negative and 73% in Gram positive. The antibiotic effectiveness in enterobacteria has been
greater than 79 % in S. aureus greater than 50 %.
