[spa] Introducción
Una de las complicaciones más frecuentes de la prostatectomía radical es la disfunción eréctil, que consiste en la incapacidad para lograr y mantener una erección suficiente para mantener relaciones sexuales satisfactorias. Esta disfunción ocurre como consecuencia de la neuropraxia y apoptosis del músculo liso debido a la cirugía, afectando a la calidad de vida. Existen diversos tratamientos, aunque los invasivos tienen muchas complicaciones. El objetivo de este estudio es determinar qué tratamiento conservador (fisioterapia o psicología) es más eficaz para tratar la función eréctil y calidad de vida de esta disfunción.
Estrategia de búsqueda bibliográfica
Se ha realizado una búsqueda bibliográfica de los últimos 10 años con descriptores MeSH y palabras clave en las bases de datos Pubmed, Biblioteca Cochrane, PEDro, PsycARTICLES, PsycINFO, EBSCOhost y Biblioteca Virtual en Salud (BVS).
Resultados
Se han obtenido 15 artículos que evalúan la eficacia del tratamiento fisioterapéutico y psicológico en la función eréctil o calidad de vida, así como de su combinación. El tratamiento más estudiado ha sido el entrenamiento de suelo pélvico, siendo los otros estudiados por entre uno y dos estudios. En cuanto a las variables, la función eréctil ha sido medida por el IIEF y la calidad de vida por diferentes escalas.
Conclusiones
En base a la evidencia científica actual, el principal tratamiento para la recuperación de la función eréctil es el tratamiento de fisioterapia, en concreto, el entrenamiento muscular de suelo pélvico precoz, con nivel de evidencia Ib y grado de recomendación A. En cuanto al tratamiento de la calidad de vida, no se pueden sacar conclusiones sobre cuál es más eficaz. |

One of the most frequent complications in radical prostatectomy is erectile dysfunction, which is the permanent incapacity to maintain an erection sufficient for satisfactorily sexual relationships. This dysfunction occurs in consequence of neuropraxia and apoptosis of smooth muscle due to the surgery, affecting quality of life. There are different treatments, but the invasive ones have many complications. The objective of this study is to determine which conservator treatment (physiotherapy or psychology) is more effective to treat the erectile function and the quality of life in this dysfunction.
Bibliographic strategy
It is realized a bibliographic search in the last 10 years combining the pathology with the treatments in Pubmed, Biblioteca Cochrane, PEDro, PsycARTICLES, PsycINFO, EBSCOhost and Biblioteca Virtual en Salud (BVS) databases.
Results
15 articles had been obtained, which evaluate the efficacy of physical therapy or psychological treatment in erectile function and quality of life, as well as its combination. The most studied treatment has been pelvic floor muscle training, and the others ones where studied with one or two articles. The variables have been measured by the IIEF in case of erectile function and by different scales in the case of quality of life.
Conclusions
Based on the available evidence, the main treatment for the erectile function is the physiotherapist treatment, specifically early pelvic floor muscle training, with level of evidence Ib and grade of recommendation A. For the treatment of quality of life, no conclusions can be done. |
