This discrimination causes the stress of minorities and multiple barriers in a system that does not contemplate them, a cisheteronormative system. All this has negative effects on the health ofthe LGBTQI people, and especially ontheir mental health. Within the health system we find barriers in all dimensions of the right to health.•Objectives The objective is to explore the scientific evidence on health care for non-cisheteronormative people from the description of their experiences and needs to identify the aspects to be improved and to be able to offer quality care.•Search strategyand results A bibliographic search is carried out in PubMed and Cochrane based on the keywords; LGBTIQ people, healthcare systemand healthcare personnel. 1059 articles are obtained, 47 are read in full and 25 articles are included.
27•Discussion For LGBTIQ people these barriers are forms of discrimination by the health system; invisibility, pathologization, lack of respect and skills to take care of their health, non-inclusive environments and language, etc. These situations can be found in all areas of the health system and by all professionals.A good training of health and non-health personnel and a change in the system and its values are key to face the health disparities of the LGBTIQ community. Cultural competence, personal and social awareness, competencies and clinical skills are the basis for competent health personnel. In addition to offering a less regulatory system and aimed at achieving the highest level of health.•Conclusion The health system has an essential role to end the health disparities of the LGBTQI community. Staff training and a deconstruction of the system is essential to break the norm and embrace diversity. And this goes beyond the health system and the LGBTIQ community. Standards affect everyone and the healthcare system is not the only thing that creates disparities, healthcare doesn’t end in the healthcare system
