[cat] Quan té lloc una fractura o patologia en el teixit ossi, és important que la regeneració del teixit sigui ràpida i completa. Convencionalment, aquests problemes, on l’organisme no és capaç de regenerar el teixit danyat per ell mateix, han estat tractats principalment amb cirurgia, amb trasplantaments ossis del mateix pacient, substitutius del teixit ossi o amb cèl·lules mare. Tots aquests presenten alguns problemes, com la limitació en el seu ús, possibles infeccions, rebuig per part de l’organisme, formació de tumors o provocar efectes negatius en el nínxol de cèl·lules mare. Recentment, s’està desenvolupant una nova opció basada en vesícules extracel·lulars (EVs), aquestes estan implicades en la comunicació intercel·lular, la qual cosa pot ser utilitzada amb l’objectiu de reparar defectes ossis mitjançant la regulació de cèl·lules i senyals pròpies del metabolisme ossi. Aquestes EVs normalment s’obtenen a partir del secretoma de les cèl·lules mare ja que aquestes tenen grans propietats proliferatives, perfectes per a la regeneració tissular. L’efecte de les EVs pot ser potenciat si aquestes es combinen amb diversos biomaterials. Aquests serveixen com a suport per a les EVs, i es poden classificar en scaffolds, implants i hidrogels. Existeix una gran varietat de materials que es poden utilitzar per a la seva fabricació, així, per exemple els scaffolds, poden estar formats per PLA (àcid polilàctic), PLGA (àcid polilàctic co-glicòlic), PCL (policaprolactona), etc. Tant els biomaterials com les EVs es poden sotmetre a tractaments per potenciar els seus efectes regeneratius com serien les EVs tractades amb PEI (polietilèimina) o els implants de titani biotinitzats. Hi ha una constant investigació de noves combinacions de biomaterials amb EVs i de possibles tractaments en aquests per a aconseguir cada vegada efectes regeneratius més significatius. Encara que aquestes combinacions tenen molt de potencial en la medicina regenerativa, constitueix una branca de l’enginyeria tissular innovadora i emergent. On només s’han fet estudis en animals, i es requereixen molts més per poder aplicar-los en humans.
[eng] When a fracture or pathology occurs in bone tissue, a quick tissue regeneration is important. Conventionally, when the organism is not able to regenerate the tissue by itself, damages have been treated by surgery using bone transplants, bone tissue replacements, or stem cells. All of these methods, present some drawbacks, such as use limitation, infections, rejection, tumor formation or negative effects on the stem cell niche. Recently, a new option is being developed based on the use of extracellular vesicles (EVs), involved in intercellular communication, that can be used in order to repair bone defects by regulating cells and signals related to bone metabolism. These EVs are usually obtained from the secretome of stem cells since they have great proliferative properties, suitable for tissue regeneration. EVs regenerative effects can be enhanced by its combination with biomaterials. These serve as a support for EVs, and can be classified as scaffolds, implants and hydrogels. A huge variety of materials can be used for their fabrications, for example, scaffolds, can be made by PLA, PLGA, PCL, etc. Both, biomaterials and EVs can be subjected to treatments to enhance their regenerative effects such as PEI-treated EVs or biotinylated Ti implants. There is a constant research of new combinations of biomaterials with EVs to achieve more and more significant regenerative effects. Although these combinations have a lot of potential in regenerative medicine, it constitutes an innovative and emerging branch of tissue engineering ehere only animal studies have been done, and many more are required before being applied to humans.
