[spa] El trastorn d'estrès posttraumàtic és una trastorn mental desencadenat davant un
esdeveniment traumàtic, com la vivència d'una malaltia crítica i d'un ingrés en la unitat
de cures intensives. El pacient crític experimenta factors estressants físics i psicològics,
que inclouen complicacions i esdeveniments adversos, i que contribueixen en la
incidència del TEPT. El seu diagnòstic inclou símptomes d'intrusió, d'evitació,
fisiològics, d'alteració cognitiva i de l'estat d'ànim. Entre les intervencions
terapèutiques, els diaris d'UCI són una estratègia potencial i cost-efectiva. Els diaris
d'UCI registren les situacions crítiques, les vivències personals i els episodis de
recuperació del pacient crític. L'objectiu general d'aquesta revisió bibliogràfica és
analitzar l'evidència científica disponible sobre l'impacte de l'ús dels diaris d'UCI en els
signes i símptomes del TEPT. Els objectius específics són descriure les experiències, els
efectes i les maneres d'aplicació en el pacient crític amb TEPT. S'ha realitzat una
recerca bibliogràfica en PubMed, Cochrane Library Plus i EBSCOHost entre els anys
2011-2021. Després de l'aplicació dels criteris d'inclusió i d'exclusió, i la lectura crítica
dels documents, es van seleccionar 20 articles científics per a la seva revisió.
L'evidència analitzada indica que l'ús dels diaris d'UCI en el pacient crític amb TEPT té
un impacte positiu en l'experiència tant dels usuaris com dels seus familiars i
professionals de vigilància intensiva: faciliten l'enteniment del pacient sobre les seves
vivències, l'acompanyament dels seus afins i el treball en equip del personal sanitari. La
majoria dels estudis indiquen una reducció d'alguns símptomes del TEPT mitjançant el
seu ús, mentre que només alguns d'ells indiquen una reducció del quadre diagnòstic
complet. La varietat de les seves maneres d'aplicació és una limitació important, i
requereix el seguiment d'unes pautes respecte a la seva estructura, els seus continguts i
el respecte per la confidencialitat del pacient
[eng] Post-traumatic stress disorder is a mental disorder triggered by a traumatic event, such
as the experience of critical illness and admission to the intensive care unit. The
critically ill patient experiences physical and psychological stressors, including
complications and adverse events, which contribute to the incidence of PTSD. Its
diagnosis includes symptoms of intrusion, avoidance, physiological, cognitive and
mood disturbance. Among therapeutic interventions, ICU diaries are a potential and
cost-effective strategy. ICU diaries record critical situations, personal experiences and
recovery episodes of the critically ill patient. The overall objective of this bibliographic
review is to analyze the available scientific evidence on the impact of the use of ICU
diaries on the signs and symptoms of PTSD. The specific objectives are to describe the
experiences, effects and modes of application in the critically ill patient with PTSD. A
literature search was conducted in PubMed, Cochrane Library Plus and EBSCOHost
between the years 2011-2021. After application of the inclusion and exclusion criteria
and critical reading of the documents, 20 scientific articles were selected for review.
The evidence analyzed indicates that the use of ICU diaries in the critically ill patient
with PTSD has a positive impact on the experience of both users and their families and
intensive care professionals: they facilitate the patient's understanding of their
experiences, the accompaniment of their relatives and the teamwork of the healthcare
staff. Most studies indicate a reduction of some PTSD symptoms through their use,
while only a few of them indicate a reduction of the entire diagnostic picture. The
variety of its modes of application is an important limitation, and requires the following
of guidelines regarding in terms of structure, contents and respect for patient
confidentiality.
