Implementación de la Fisioterapia de primer contacto en la atención primaria para el dolor lumbar. Un estudio mixto

Abstract

[spa] Introducción y justificación. El dolor lumbar (DL) es una de las principales causas de discapacidad, costes sanitarios y pérdida de trabajo en el mundo, puesto que es el problema de salud musculo-esquelético (ME) más común. Los médicos de familia (MF) son el primer punto de contacto más común en el sistema sanitario para las personas con DL y la evidencia sugiere que el colectivo de fisioterapeutas puede aplicar eficazmente las recomendaciones de las directrices de atención primaria (AP) para el DL. Se necesitan estudios que determinen el impacto clínico, en el sistema sanitario y en la sociedad de un modelo de AP que integre fisioterapeutas en la primera visita para personas con DL. En los últimos años se está estudiando en el contexto internacional la FPC. Objetivo general. Determinar si es costo-efectivo implantar la FPC en AP para usuarios con DL en comparación con el proceso de atención habitual. Metodología del estudio. El estudio se desarrollará en tres fases: en primer lugar, se llevará a cabo un ensayo controlado aleatorizado donde se evaluarán los costes de la intervención la FPC, la efectividad de la intervención en intensidad de dolor y la calidad de vida. El muestreo será aleatorizado y la población de estudio serán los usuarios de la red pública de atención primaria de la isla de Mallorca. La segunda fase, de naturaleza cualitativa, será diseñada desde un enfoque socio crítico y desarrollada mediante entrevistas en profundidad y grupos focales; serán analizadas mediante análisis de discurso. Por último, se analizará el impacto social de la intervención con un SROI. Se solicitará evaluación y aprobación por parte del comité ético de las Illes Balears (CEIB). Implicaciones i potencialidades. Los resultados del presente estudio tienen el potencial de mejorar la efectividad de las intervenciones en DL en AP y reducir los costes sanitarios derivados de la intervención, además de facilitar el acceso y la equidad de la atención en fisioterapia.

Low back pain (LBP) is one of the leading causes of disability, healthcare costs and work loss in the world and it is the most common musculoskeletal health problem. Family physicians (GPs) are the most common first point of contact in the health care system for people with LBP. Evidence suggests that physiotherapists can effectively implement the recommendations of primary care (PC) guidelines for LBP. Studies are needed to determine the clinical impact into the health care system and into society of a PC model that integrates physiotherapists in the first visit for people with LBP. In recent years, first contact physiotherapy (FCP) in PC is being studied in the international context, indicating its cost-effectiveness. General objective. To determine whether it is cost-effective to implement FPC in PC for users with LBP in comparison with the usual care process. Study methodology. The study will be carried out in three phases. First of all, a randomized controlled trial will be conducted in order to evaluate the costs of the CPF intervention, the effectiveness of the intervention on pain intensity and the quality of life. The sampling will be randomized and the study population will be the users of the public primary care network of the island of Mallorca. The second phase, of qualitative nature, will be designed from a socio-critical approach and it will be developed through in-depth interviews and focus groups; they will be analyzed through discourse analysis. Finally, an analysis of the costs and social impact of the intervention will be carried out with an SROI. Evaluation and approval by the Balearic Islands Ethics Committee (CEIB) will be requested. Implications and potential. The results of the present study have the potential to improve the effectiveness of interventions in LBP in PC, reduce the health costs derived from the intervention, in addition to facilitating access and equity of care in physiotherapy.
