[cat] La infecció per Clostridioides difficile (CDI) és reconeguda com a la causa etiològica més diagnosticada de diarrea nosocomial, provocant una alta morbiditat i mortalitat. La taxa de resistència de C. difficile als antimicrobians està creixent ràpidament a tot el món. Entre ells s’hi troben els utilitzats com a tractament principal de la CDI, el que evidencia la necessitat de realitzar investigacions per conèixer els mecanismes de resistència cap a aquests antibiòtics i així poder trobar solucions. A aquest treball s’ha dut a terme un estudi de susceptibilitat a diversos antibiòtics fets servir en el tractament de la CDI (vancomicina, metronidazole, rifampicina i tigeciclina) per part de soques toxigèniques aïllades de pacients hospitalitzats a l’Hospital Universitari Son Llàtzer entre l’any 2017 i 2021, per tal de veure canvis en el patró de resistències de dit microorganisme. La metodologia seguida ha estat la de difusió en disc i epsilometria (E-tests). Els resultats de les 130 soques estudiades han mostrat sensibilitat a vancomicina, metronidazole, rifampicina i tigeciclina. A més, s’ha observat una elevada prevalença de CDI a persones majors de 65 anys, així com una tendència a presentar CMI superiors. També s’ha comprovat l’eficàcia de mètodes senzills com la difusió en disc i l’E-test per realitzar proves rutinàries als laboratoris. Per tot això, la realització d’investigacions similars seria interessant, ja que ajudarien a conèixer com evoluciona la patogènia de C. difficile, a proporcionar nous objectius i idees per a desenvolupar antibiòtics efectius i a aplicar mesures per a prevenir la CDI.
[eng] Clostridioides difficile infection (CDI) is recognized as the most diagnosed etiological cause of nosocomial diarrhoea, causing high morbidity and mortality. The rate of C. difficile resistance to antimicrobials is growing rapidly worldwide. Among them there are those used as the main treatment for CDI, which shows the need for research to understand the resistance mechanisms to these antibiotics and thus be able to find solutions. In this work, a susceptibility study was performed on various antibiotics used in the treatment of CDI (vancomycin, metronidazole, rifampicin and tigecycline) by isolated toxigenic strains from patients hospitalized in the university hospital Son Llàtzer between 2017 and 2021, in order to see changes in the resistance pattern of this microorganism. The methodology followed was disk diffusion and epsylometry (Etests). The results of the 130 strains studied showed sensitivity to vancomycin, metronidazole, rifampicin, and tigecycline. Also, a high prevalence of CDI in people over 65 years of age has been observed, as well as a tendency to present higher MIC. The effectiveness of simple methods such as disk diffusion and E-test to perform routine laboratory tests has also been shown. Therefore, conducting similar research would be interesting, as they would help to understand how the pathogenesis of C. difficile evolves, to provide new goals and ideas for developing effective antibiotics, and to implement measures to prevent CDI.
