[eng] Background One of the most frequent causes of visits to the emergency room for surgical pathology are perineal abscesses, and supralevator abscesses, it is a rare subtype but necessary to suspect, to make a correct diagnosis and treatment. Clinical case: Male patient comes to the emergency room with proctalgia and fever. Abdominal-pelvic CT is performed and a supralevator abscess is oriented, performing surgical debridement. Conclusion: Given the suspicion of a supralevator abscess, it would be indicated to perform a pelvic MRI for a correct diagnosis, but a pelvic CT scan is feasible. The drainage route will depend on the origin and evolution of the abscess
