[spa] Introducción. La esclerosis lateral amiotrófica (ELA) es la forma más frecuente
de enfermedad progresiva de motoneurona y se caracteriza por una parálisis progresiva
y un fallo respiratorio que conduce a la muerte entre el tercer y quinto año después del
diagnóstico. La prevalencia de desnutrición varía en los estudios entre un 16% y un
53%. Un IMC bajo afecta negativamente la progresión de la enfermedad y por tanto la
supervivencia en pacientes con ELA. A pesar de que la gastrostomía es un
procedimiento sin grandes complicaciones y que se ha demostrado que niveles más
elevados de IMC se relacionan con mayor supervivencia, actualmente la nutrición
enteral a través de gastrostomía se coloca cuando la desnutrición es muy evidente.
Objetivos. Analizar el impacto de la nutrición enteral precoz a través de una
gastrostomía radiológica sobre el peso y el IMC de los pacientes y su calidad de vida,
evaluar la eficacia del tratamiento en la mortalidad a 12 meses y describir las
complicaciones derivadas de la técnica. Material y métodos: ensayo clínico
multicéntrico poblacional aleatorizado de diseño paralelo, fase IV no ciego. El grupo de
intervención recibió nutrición enteral nocturna de forma precoz con una fórmula
hipercalórica hiperproteica con fibra y el grupo control siguió con recomendaciones
dietéticas y suplementación por vía oral. La duración del estudio fue de 12 meses
durante los cuales se realizaron varias visitas en las que se realizó una valoración
nutricional y evaluación de la calidad de vida. Resultados: En el análisis descriptivo de
las variables cuantitativas se expresan los datos como medias (±desviación estándar).
Los dos grupos serán comparados utilizando la X2 del test exacto de Fisher para las
variables discretas. Para la comparación de dos medias independientes con varianza
homogénea o iguales se realizará la T de Student. Se llevará a cabo un análisis
multivariante para detectar los factores asociados al mantenimiento o aumento del peso,
la calidad de vida y la mortalidad. |

The prevalence of malnutrition varies in studies between 16% and 53%. Low BMI
adversely affects disease progression and therefore survival in patients with ALS.
Although gastrostomy is a procedure without major complications and it has been
shown that higher levels of BMI are associated with greater survival, currently enteral
nutrition through gastrostomy is placed when malnutrition is very evident. Goals. To
analyze the impact of early enteral nutrition through a radiological gastrostomy on the
weight and BMI of the patients and their quality of life, to evaluate the effectiveness of
the treatment in the mortality at 12 months and to describe the complications derived
from the technique. Material and methods: randomized, multicenter, parallel-design,
non-blind phase IV multicenter clinical trial. The intervention group received early
nocturnal enteral nutrition with a hypercaloric hyperprotein formula with fiber and the
control group continued with dietary recommendations and oral supplementation. The
duration of the study was 12 months during which several visits were made in which a
nutritional and quality of life assessment were performed. Results: In the descriptive
analysis of the quantitative variables the data are expressed as means (± standard
deviation). The two groups will be compared using Fisher's exact X2 test for discrete
variables. For the comparison of two independent means with homogeneous or equal
variance, T Student will be performed. A multivariate analysis will be done to detect
factors associated with maintenance or weight gain, quality of life and mortality. |
