Evaluación de la técnica de recogida ex útero de sangre de cordón umbilical (SCU) para su almacenamiento como fuente de progenitores hematopoyéticos para trasplante

[spa] INTRODUCCIÓN: La sangre de cordón umbilical es una fuente de progenitores hematopoyéticos utilizada para tratar distintas enfermedades hematológicas y genéticas. El método de recogida in útero es el más utilizado en la actualidad, pero el método ex útero tiene una serie de ventajas que pueden ser útiles en algunas maternidades. METODOLOGÍA: Se diseña una técnica de recogida de SCU ex útero y se aplica por parte de una estudiante de sexto de medicina en el Hospital Universitari Son Espases. Se recogen variables obstétricas y de resultado de las donantes y las muestras obtenidas. También se analizan los datos sobre recogida de SCU en los últimos 3 años en Baleares. RESULTADOS: Se realizan un total de 7 recogidas de SCU mediante la técnica ex útero en el Hospital Universitari Son Espases. De ellas, solo una obtiene una celularidad suficiente para ser apta para continuar su procesamiento. Se observa buena correlación entre el peso de las muestras obtenidas y el número de células nucleadas totales. Analizando los datos históricos de recogida de SCU en Islas Baleares, se observa que el pinzamiento de cordón tardío no debería ser criterio para no realizar una recogida. CONCLUSIONES: Es fundamental encontrar métodos para optimizar las recogidas de SCU. Introducir a personal experto en recogida de SCU en las maternidades puede fomentar la implicación del servicio en la donación

[eng] BACKGROUND: Umbilical cord blood is a source of hemopoietic progenitors used for the treatment of different hematologic and genetic disorders. In utero collection is the most used technic, but the ex utero method has some advantages that may be useful for some maternities. METHODS: An ex utero method is designed and applied by a sixth year medical student for umbilical cord blood collection in the Hospital Universitari Son Espases. Obstetrical and outcome data were collected from the donors and the samples obtained. Historical data of the cord blood collection in the Balearic Islands during the last three years is analyzed. RESULTS: Cord blood units were collected from 7 mothers using the ex utero technic at the Hospital Universitari Son Espases. Only one of them obtained enough nucleated cells to be suitable for continuing its processing. A good correlation between the weight and the cellularity of the samples is shown. Analyzing the historical data of the cord blood collection in the Balearic Islands, it is shown that a delayed umbilical cord clamping should not be a restrictive criterion for not collecting an umbilical cord blood sample. CONCLUSIONS: It is essential to find methods to optimize the umbilical cord blood collection. Introducing experienced personnel in the maternities may help increase the service commitment with the donation.
